Fee Schedule:

New patient visit $120 (up to 75 mins)
Follow-up visit $90 (60 mins)

Students (High school and up)
New patient visit $100 (up to 60 mins)
Follow-up visit $75 (45-60 mins)

Kids (0-14)
New patient visit $80 (45 - 60 mins)
Follow-up visit $50 (30 mins)

* Payment is due at time of service. We accept cash, checks, and credit card.
* Please be advised that $40 will be charged for missed or cancelled appointments unless 24 hours notice is given.
* A $25 fee will be charged for any returned checks.
* We accept Worker's Compensation cases, no fault car accident cases, Veteran's Affair, major insurance (Kaiser, HMSA, HMAA), and medicare advantage (United Health, Humana, and Ohana). ​
